The Kenmore-Tonawanda Union Free School District has implemented a new policy regarding the use of hand sanitizers in its schools. Parents who do not wish for their children to use alcohol-based hand sanitzers may alert the school by filling out a form, available here.

Student Requirements
Upon admittance to COHS, students must provide proof of immunization as required by New York State.  Information about immunization requirements can be found here.

New York State requires all 9th grade boys to be screened for scoliosis,  11th grade to be screened for vision and hearing, and all new students screened for vision, hearing, and color perception in the health office.  All children who are screened for vision will receive the results by mail per New York State Guidelines.

As of July 1, 2018 New York State law requires a health examination for all students entering the school district for the first time and when entering Pre-K or K, 1st, 3rd, 5th, th, 9th, and 11th grade.

Parents will be notified in writing if results show apparent deviations from normal in any of the above screenings.

Students who wish to participate in school-sponsored sports activities at COHS must complete and submit a health form prior to beginning tryouts.  This two-page form is available for download below.

First Aid
Staff may provide first aid (initial care) only. This does not include diagnosis and/or treatment or changing dressings on existing wounds.

Emergency Contact Information
Please keep all emergency contact information current and on file in the health office. Students should bring home the emergency information cards on the first day of school. Please include as much information as possible, work numbers, cell phones, pagers and at least two (2) parent substitutes to be contacted if you can not be reached in an emergency. In high school we do at times have severe gym injuries or accidents. We need to have quick access to a parent so you can determine which hospital you would like your child sent to.

Health Records
Individual health records are maintained for each child. These include immunizations, physicals, screenings and physical education excuses. 

A copy of your student's immunization record will be sent to all seniors prior to graduation.

Working Papers
Students needing working papers must submit required documentation.  The physical fitness certification form required for working papers is available in the Health Office and can be downloaded here.
